Javascript question...

I have never used &#106avascript. But now I need to write an HTML page where the user has to fill in a form. No big deal so far, but I want the form to have a few questions on them, in the form of the like: 
1. Question 1
A. Bla
B. Bla
C. Bla
D. Bla

2. Question 2
etc.
The A,B,C,D answer are radiobuttons. My question, how do I make a simple page where I can have several radio buttons + text and a submit button? If you show me how I can see what radio button is "checked", how I can group them, and how I can load the second page or display a messagebox when there isn''t a group checked I would be happy.
