I can't get there, so I hope there is someone smarter than me. Using a windows boot cd, during boot, we see: Press any key to boot from cd... and then: Setup is inspecting your computer's hardware configuration... My question is, what exe is running at this time, or is it all boot? What is the first exe to get called?

The first part of the setup is run by winnt32.exe I believe.

WINNT.EXE

WINNT32.EXE will only run from within Windows, IIRC.
Actually... I'm not sure. If you create a DOS boot disk you have to use WINNT.EXE to get installing, maybe the CD has a basic Windows boot? No idea.

After disassembling the boot sector, I noticed the code called the binary execution file SETUPLDR.BIN, which in turn seems to be the program that runs the Windows installation.

Problem is, why would like to get a menu here, to be able to select other options before Windows is run. Current sequence is:

Boot -> SETUPLDR.BIN -> Windows Setup

...and I would like it to be something along the lines of:
Boot -> My Menu -> (..) (Windows/ HD Health App/ etc...)

Does anyone have any bright ideas?

I guess you could write your own loader which displays the menu and then loads that .BIN file at the appropiate memory location. Rename your loader to SETUPLDR.BIN and rename SETUPLDR.BIN to something else so you don't need to change the boot sector.
You need to check what the boot sector does as far as protected mode is concerned and the memory location at which it loads SETUPLDR.BIN. I'm guessing that SETUPLDR.BIN is a stub which puts your CPU into protected mode and loads winnt32.exe or winnt.exe and runs it so SETUPLDR.BIN is proably starting in real mode thus you shouldn't have much work to do on your own menu-loader.
