Simple yet effective method which I've tried is: big sphere and a plane. Which I was very surprised to see in Shiny's Sacrifice demo when I downloaded it yesterday.

Since you've got a skybox, you can probably do without the sphere. Take a plane and place it at an 'optimum' level above your play area. I say optimum in quotes because it takes a bit of playing with to make it visually work with your environment.

Then scroll an alpha-blended image on it with all Diffuse colors = zero. Alter the Diffuse alpha property to adjust the amount of transparency. Check out this screen shot from my demo:

(you'll have to zoom in to get a decent shot).

The black sky with stars is on the dome. The stratified looking sky is an alpha-blended texture scrolling on a plane.

Anyone know how to have both D3D fog enabled and a skybox visible
in the background? Since the skybox is drawn with the z-buffer
disabled, the fog never draws over the skybox. But the z-buffer
needs to be disabled to draw the skybox properly in the first place.

You can''t fog a skybox - if you do, it will become painfully obvious that you''re trapped inside a big cube. If you want your sky to display fog, you need to render it into the skybox textures (e.g. with Bryce). You should disable fogging while rendering the sky.
