Lets say I have a fullscreen app. When I create surfaces in video memory and my app loses focus some of the surfaces become garbage. But if I create them in system memory the surface stays intact. Is this normal or am I doing something wrong?

Read up on Lost Devices.

Basically there are certain actions (such as losing focus when you have a fullscreen window) that cause a device to become lost. When a device becomes lost it must be reset all resources in video memory must be recreated while those in system memory don't have to be recreated. See D3DPOOL for more precise info on what resources have to be recreated when a device is lost.
